I understand that Waybright Academy may enroll my student in courses provided by the ASU Universal Learner Program. These courses are a selection of first-year college courses, delivered online and designed to allow students a way to earn college credit at a fraction of the cost of traditional college credits.

I understand that Waybright Academy may, in its sole discretion, enroll my student in courses provided through the ASU Universal Learner Program. I give permission for Waybright Academy to enroll my student in these courses, and to disclose to the ASU Universal Learner Program the personal information of my student necessary for registration in the program. This information includes a copy of my student’s driver’s license or state issued photo ID, name, date of birth, address, phone number, and demographic information.

I understand that my student will receive an ASURITE ID to sign in to the ASU Universal Learners Program, and my student will use the ASURITE ID for any Arizona State University (“ASU”) courses taken during their educational career. I understand that by enrolling in the ASU Universal Learners Program, I am also agreeing to the Open Scale Terms of Use as defined by ASU. Furthermore I understand that it is my responsibility to locate, read, understand and abide by all of ASU’s Open Scale Terms of Use, at all times and as they relate to the ASU Universal Learner program as part of this agreement. I understand that there are costs associated with the ASU Universal Learners Program and that those costs may change. I authorize Waybright Academy in its sole discretion to pay these costs, including upgrade fees and Course Completion fees, on behalf of my student. I understand that Waybright Academy may pay these fees or request funds to pay these fees before any payments are made to ASU and in the event my student obtains an overall GPA at ASU of 3.00 or better. By paying the Course Completion fee, the course and credit hours will become part of my student’s permanent college transcript.

I further understand that Waybright Academy may, in its sole discretion, withdraw my student without notice from a course, based on its determination of the best needs of my student. Withdrawing from an ASU Universal Learner course does not place the course and credit hours on my student’s permanent college transcript. No refunds will be given in the event of a withdrawal. My student may still receive credit on their high school transcripts for the course.

During the course of enrollment personally identifiable information may be disclosed to Waybright Academy that is covered by the U.S. Family Educational Rights and Privacy Act, 20 U.S.C. § 1232g and its implementing regulations (“FERPA”). This includes, but is not limited to, information about a student's academic standing, disciplinary issues and financial obligations to ASU and the Universal Learners Program. I consent to this disclosure.

Your agreement to use an electronic signature with us for any documents will continue until such time as you notify us in writing that you no longer wish to use an electronic signature.
